Lesions begin preferentially at junction points and at sites of mechanical stress in the arteries, where local endothelial metabolism is chronically challenged. At very initial stages one detects morphological changes characterized by a cytoplasmic accumulation of cholesterol esters, giving a typical aspect of "foam cells". At this stage of fatty streaks, lesions consist of a clustering of sub-endothelial or intimal foamy macrophages and of T lymphocytes retained in the extracellular matrix. Then a vicious circle establishes in which activated cells proliferate and recruit more smooth muscle cells from the media.

In the second case, several genes may interact to different extent, often modulated by environmental factors to give rise to lipid disorders, generally more variable in expression and of later onset. Contrary to general knowledge monogenic dyslipoproteinemias are not rare [117]. They could account for 50 to 60% of dyslipidemia observed in patients with coronary heart disease [\\8]. Some of them result from defects on single genes known to encode for candidate genes of lipoprotein metabolism (table 1-3).
